W-4 Employee Withholding Allowance Certificate 
The W-4 is required of each employee to determine the appropriate federal income tax deduction to be withheld from the employee’s pay each pay period. The filing status and number of exemptions declared remain in effect unless and until a new W-4 is submitted by the employee, and will supersede any previous W-4(s).

Employee Name/Address/Directory Information Change Form 
The Employee Information Change form is required to effect a change in name, address, and/or staff directory inclusion information.
